Issue of Certificates of Probable Age

  1. Certificates of probable age are issued to whose birth cannot be registered.
  2. Certificates of probable age are issued to,
    1. Resident children of a children's home approved by the government.
    2. Children who are under the guardianship of persons other than children's homes approved by the government
  1. Documents to be submitted by the resident children of a children's home approved by the government.
    1. Affidavit of the matron of the children's home
    2. Medical certificate issued by a government medical officer determining the age of the child
    3. Other documents containing information of the child (if any)
  1. Documents to be submitted by the children other than residents of a children's home approved by the government together with the declaration;
    1. Affidavit of the guardian
    2. Documents in proof of the guardianship of the guardians regarding the child concerned
      • Grama Niladhari certificate or
      • Certificate of the probation and Childcare Service Officer
      • Certificate from the Social Services Department
      • Certificate issued by the Inspector of Police
    3. Medical certificate issued by a government medical officer determining the age of the child
    4. Health Development Record
    5. Copy of the school admission register if the child attends school
